Treatment of the ectopic ACTH syndrome is difficult. Surgical removal of the ACTH-producing tumour is desirable but difficult in most cases. Effective chemotherapy and/or radiotherapy of the neoplasms may produce relief from the syndrome. The metabolic abnormalities that are induced by hypercortisolism can be controlled by either surgical bilateral adrenalectomy or medical treatment with metyrapone, o,p-DDD, ketoconazole, or aminoglutethimide.
